Adult Membership $351 saving/ year
- 1 Comprehensive exam
- 2 Dental cleanings

- 1 Set of (4) BW x-rays
- 1 Panoramic x-ray every 5 years.
- 1 Emergency exam.
- For patients with healthy gum
Perio Membership $501 saving/ year
- 1 Comprehensive exam
- 3-4 perio maintenance cleanings
- 1 Dental exams
- 1 Set of (4) BW x-rays
- 1 Fluoride treatment
- 1 Emergency exam.
- For patients with gum disease
Child Membership $394 saving/ year
- 1 Comprehensive exam
- 2 Dental cleanings
- 1 Dental exams
- 1 Set of (4) BW x-rays
- 1 Fluoride treatment
- 1 Emergency exam.
Bucktown Smile Club Membership Discount Dental Plan Cost
Adult membership One lifetime activation fee of $89. 11 Monthly membership dues of $28 starting one month after join date.
Child membership One lifetime activation fee of $89. 11 Monthly membership dues of $28 starting one month after join date.
Perio membership One lifetime activation fee of $89. 11 Monthly membership dues of $54 starting one month after join date.
Guidelines
- The Bucktown Smile Club is not insurance and is not qualified health plans under the Affordable Care Act.
- Bucktown Smile Club payments must be current to receive the services and discounts included in the membership plan.
- No refunds given if patient does not utilize program benefits for the year
- Membership Benefits can only be used at Bucktown Dental Associates.
- Bucktown Smile Club is a discount plan and cannot be combined with conventional dental insurance or any other discount plan or offer.
- Benefits do not transfer from one member to another
- If your treatment requires a referral to a specialist, you can't use the plan discount.
- Should there be any treatment needed following an injury where a lawsuit is involved(ie. Medical care, disability, workman’s comp, etc.), this plan cannot be used
- Payments for services are due at time of service to receive the discounted rate.
- If you choose to extend your payment for treatment through a third party finance company (ie. CareCredit), the discount will be reduced by 10%.
- Bucktown Smile Club plan is subject to change annually; however, your agreement will be honored for the full year of your individual enrollment
