To directly schedule an appointment from our website, please fill out the form below. 

*PLEASE READ PRIOR TO SCHEDULING*:

Select 'Skin Cancer Screening' if you are a new OR current patient scheduling a general skin cancer screening. 

Select 'New Patient - Focused' if you are new to our practice and have a specific concern to address.

Select 'EST PT - Focused' if you have been seen previously and have a new concern to address or a follow up appointment on existing issue.

For a focused check, in the "Reason for Visit" box, please put the single, main concern you'd like to address at this appointment.

NEW PATIENTS:

You will receive an email within 24 hours of scheduling your appointment. Please follow the link to activate your "Patient Portal" and complete our electronic 'Check-In.' 

If you are unable to complete the portal check-in, please arrive 10-15 minutes before your scheduled time slot. We are unable to confirm insurance benefits unless we have the completed check-in prior to your appointment date.
