Quakertown Pediatrics welcomes new patients. Choosing the right pediatrician for your child is one of the most important decisions you will make. By selecting Quakertown Pediatric, you can feel confident and comfortable that you have made the right decision.

Call Hour

You can reach our providers every day except Sunday between 8 and 9 am.  Call (215) 536-1915.  You can ask any questions or decide if your child needs an appointment.  Our providers are also available after hours through our answering service.

What to bring to your first visit?

We work hard to ensure that your child receives prompt medical care in a comfortable, child-friendly office setting. To help expedite the first appointment, we ask that you please bring your child's immunization records and be prepared to complete paperwork regarding your family medical history, your child's medical history, and insurance information.

If you are new to our pediatrics practice, please download the listed forms and complete before your first office visit. Please bring ALL forms to your first appointment. If you are a current patient with new information, please download all of the forms to bring to your next appointment.

What to Expect

On the day of your child's first visit to our office, we recommend arriving a few minutes early. This allows extra time for finishing up any registration forms and ensures you have plenty of time to get acquainted with our staff and office. We look forward to meeting you and joining with you to help your child maintain optimal health. 

Transfer of Medical Records

Please complete the release of medical records form and submit to our office. You can email, fax or mail the form. There is a charge of a maximum of $20.00 per patient for your medical records. For current visits you can download your child's immunization records, growth charts and office visit summaries through our patient portal at no charge. You must however be registered for our patient portal for access.

When you transfer medical records because you are over the age of 21 or we no longer take your insurance we will waive the fee. You can contact Sue Harris at (215) 536-1915 for any further questions.
