House Calls

Dr. Blakeslee has been providing House Calls to home bound patients since 2001. Dr. Blakeslee prides himself on his ability to give the same quality of care that would be found in his offices, right down to his sterilized equipment.

As we know it is difficult for many patients that are home bound and still need foot care. We do offer assistance with rides to and from the office, but if this is still not an option, you can apply for House Calls. House Calls are available for patients that meet certain criteria. Some insurances do not cover House calls. Are you or your family member home bound? (Meaning you or they are not able to leave the house without assistance.) If you are able to leave your home for other doctors’ appointments, we can help arrange transportation to our offices and are not eligible for House Calls.

If you have been accepted as a House Call patient, below are the polices. Our office has strict guidelines for House Calls since they are in high-demand and limited space is available.

The items below must be received by our office before an appointment can be arranged.

  • House call Packet
  • HIPAA
  • Paperwork must be completed
  • Copy of Insurance Card or Cards
  • Copy of Photo ID
  • Singed agreement for cancellation
  • Contact information for family member if overseeing care
  • Information of aides or nursing support

Emergencies

As with any emergency, call 911. Once the emergency has been stabilized, please call our office. Often times we can continue care if hospital stay is required. If care is put on hold, we will note our system to when to reactivate care.

Once paperwork is received. Our office will call and book your appointment. We currently do House Calls on Tuesday and Wednesday and determine where your location fits into our routes. We understand some patients would like to choose other times, however, Dr. Blakeslee has a very specific and planned route for efficiency and maximum patients scheduled. Request to change dates or times is not an option at this time for this service. Next our Medical Assistant will call to obtain any missing information. If your family member is not able to speak for themselves, we will need the contact name and number with the best available time for the person who is.

Next, our office will need all the information for gaining access to the location, including animals. We will confirm your appointment the day before Dr. Blakeslee’s arrival. We need to have verbal confirmation the patient will be available and ready during the window allotted. We will give a 2 hours window of Dr. Blakeslee’s arrival. If we do not get verbal commitment, we will attempt 1 more time. If still no confirmation, we will not be able to hold this house call and it will be up to you to call our office and reschedule the next available slot.

The day of your appointment, our office will call again and let you know Dr. Blakeslee will be arriving in the next 15 to 30 minutes. Please make sure to have your doors opened and ready for his arrival.

Below are the requirements for your visit:

  • Notify the front gate if you live in a gated community
  • Leave the front door unlocked
  • Be seated in a reclining chair or have a stool to put your feet up
  • Socks and shoes should be taken off
  • Area is well lit
  • Have paper towels or a regular towel ready
  • Animals should be secured
  • Have insurance cards and medications readily available for the doctor to check
  • Have your Co-pay or deductible available

Cancellations

Cancellations are sometimes unavoidable, but we ask for at least 24 hours’ notice as a courtesy to other patients who are waiting for an appointment and out of respect for Dr. Blakeslee’s time.

For any appointment cancelled in a timely manner (with more than 48 hr notice) you can be rescheduled in 2 weeks or at our earliest convenience and will be notified as soon as possible.

Any cancellations made less than 24-48 hours from your scheduled appointment time, will be added to the waiting list up to 10 weeks.

There will be a $75 fee for cancellations made the day of your scheduled appointment or if Dr. Blakeslee arrives to find no one is home. Once your cancellation fee is paid, you will be added back on the schedule in 10 weeks. You may make an appointment at any time in the office, if you need to be seen sooner. Multiple cancellations may result in termination of house calls.
