What is your phone and fax number?
Phone: 305-598-9090 and Fax: 305-598-0668. After office hours, there is a voice recorded answering service for emergent calls that will notify the doctor on call. All other non-emergency calls will be answered the next business day. Messages left in our voicemail system throughout the working hours will usually be answered the same day.

What are your office hours?
Monday to Friday 9 AM to 5 PM, lunch break from 12:00-1:00 PM.

Can I fill out paperwork beforehand?
Yes. We encourage you to download, print and complete the forms and bring them to your first appointment to save yourself time. You can find these forms in the New Patients tab.

Do I need a referral?
Depending on your insurance policy you will need referrals. It is the patient’s responsibility to know whether or not s/he will need a referral. If the referral is necessary, it is the patient’s responsibility to present it at the time of appointment.

What documents should I bring for an office visit?
You MUST bring a photo identification and insurance card. Please have a pharmacy number, current medication list, and other important documents like coronary stent cards, pacemaker ID cards, important medical records or results with you on all your visits.

Where should I park?
Parking spaces are limited but there is free valet parking at the rear of the parking lot.

Do you charge a cancellation fee?
The office charges for cancellations within 24 hours of your procedure $75 the first time, $125 the second time you cancel.

Rights and Responsibilities

Privacy/ Confidentiality:
Dr. Llaneza and the staff will protect your privacy and the confidentiality according to HIPPA and any other terms the patient wishes.  Patient’s information will not be released unless authorized to do so.

Respect:
Dr. Llaneza and the staff will treat each patient with the highest level of respect. Likewise, our patients are expected to treat the staff and Dr. Llaneza with the equivalent level of respect.

Provide all accurate information:
It is the patient’s responsibility to provide all current and accurate information for all questions including medical history.

Finance responsibility:
Patient assumes all financial responsibility of paying for services. The patient must know their insurance policy and if there are any unique exceptions this should be informed to the front desk before the appointment.
