MEDICAL RECORDS
Once your/your child’s medical records have been released to you or transferred to a new physician, you will be inactivated in our system. You will need to contact your new physician for all future services. Please have your requests to our office prior to October 10, 2022.
If you have not chosen a new physician prior to that date, we will hold the request and forward it to the company that will be retaining our medical records. We will be posting that information prior to our closing date of October 28, 2022.
PATIENT APPOINTMENTS
We will only be able to see patients with acute illnesses and Well Child Checks for patients 2 years and younger. We encourage all our patients to choose a new pediatrician to ensure there is not a disruption in your medical care. This is extremely important for medications, therapy services and home health care services.
