Appointments

When calling for a same day appointment, please leave a message on the voicemail. Leave a few details as to why you are calling, so we are able to direct your call. Please direct your call to a staff member who will help you with your appointment. We do not guarantee a same day appointment.

If the nature of the appointment is regarding a potential life-threatening event, please call 911 and/or go to the emergency room. This is especially true for those patients who have a cardiac history with symptoms of chest pain, or stroke like symptoms.

After hours phone calls

After hours phone calls for canceling or confirming an appointment should be directed to the voicemail at 440-284-1400. Other calls can best be managed during office hours.

Messages

Messages left on our voicemail should include your name(please spell your name), birth date, and return phone number. A complete message would be greatly appreciated.

Insurance and Billing

We accept most traditional insurance plans. Please contact your insurance company to verify acceptance and to insure Dr. Lender is a preferred provider on your plan. This is the patient's responsibility. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. It is the patient's responsibility to verify coverage of all procedures. If you have a question about your bill, please call our billing office at 440-327-9300.

Co-Payments and Financial Responsibilities

We accept checks, cash or credit cards(Visa, MasterCard, American express and Discover.) for co-payments and self-pay procedures. All payments are due at each and every visit. Co-payments that are not paid at the time of visit will be charged a $10 surcharge, i.e., an additional charge to the co-pay. All co-pays and remaining balances should be paid at the time of service. Services will not be rendered until patient responsibility due is paid in full. All remaining fees not collected within a 90-day period may be subject to collection. Cancellation and No-Show Policy

A 24 hour notice is requested for cancelling an appointment. You can leave a message on the voicemail to cancel an appointment. If you are sick or have an emergency the day of your appointment and wish to cancel please call the office as soon as possible. The No-Show fee will not be charged for these appointments.

The No-Show policy is for patients who make appointments, have a confirmation call made to them to remind them of an appointment and fail to call to cancel or reschedule their appointment, and do not show up for their appointment. In this instance a $25 fee will be charged to the patient. This charge is not covered by your insurance.

Prescriptions

If you need a prescription refill you will need to leave the name and strength of your medication and how often you take or use the medication. We will also need the name and phone number of your pharmacy. There is a 48-hour turnaround time on all prescription requests. If the physician is out of the office the day you call, it will have to wait until she returns of if you call on a Friday, it may have to wait until Monday.

Prescriptions will not be filled or called in until all outstanding balances are paid in full.

We appreciate if you could please plan ahead to have your prescriptions refilled, and not wait until you are almost out or completely out of your medication. When you are going out of town for an extended period of time, please give the physician sufficient time to fill the prescriptions so you will have all of your medications before you leave.

Pre-Authorization requests for medication will be submitted bi-weekly on Tuesday and Thursday afternoons. It is the patient's responsibility to obtain the phone number for the pre-authorization form. Once the pre-authorization process has been submitted please allow two (2) weeks for the insurance to review.

Annual Skin Checks

Please call for your appointment 3 months ahead as these appointments take longer and book quickly, especially early morning and afternoon appointments.

Referrals

If your insurance plan requires a referral to see a specialist, it is the patient's responsibility to obtain a referral from your primary care physician. You need to make sure you have a current referral in place prior to your appointment otherwise the office visit may not be covered.

Lab and Testing Orders

If the physician writes an order, you need to check with your insurance company to see when and where your testing can be done in order for your insurance to pay for it. Some procedures that the physician has ordered may require pre-certification or authorization by your insurance company prior to the testing being performed. It would be helpful to us if you check with your insurance company to see if a pre-certification or authorization is required.

After completing the test, the patient should receive a notice that his or her physician has reviewed the test. If the patient has not received a notice either via telephone or by mail after two weeks, please call the office.
