Flu vaccine consent form

Flu Vaccine available at HIP

We have received our first shipment of the flu vaccine and will be holding our first flu clinic Saturday September 7th from 8:30am to 12:00pm, with an additional clinic scheduled Saturday September 14th from 8:30am to 12:00pm. More flu clinics will follow and will be announced on our website.

Call our office at 720.583.4470 option 1 to schedule an appointment. Please note these flu clinics are for flu vaccines only. A separate appointment is required for any other vaccine needed. If you are interested in combining the flu vaccine with another vaccine, please schedule a nurse visit with our front desk.

If your child has a Wellness Visit soon, they can receive their flu vaccine then.

To help make these visits as efficient as possible, please download the consent form and fill one out for your family. The form can be found here

We have also received our shipment of flu vaccine for families covered by Medicaid or health share plans, from the Vaccines For Children (VFC) program. This program provides vaccines, including influenza, to practices in Colorado for families who have Medicaid insurance or no health insurance coverage.

Parents can receive flu shots at HIP. For parents who would like a flu vaccine, please let the schedulers know when you are setting up the appointment for your child.  Bring your insurance card with you so we can confirm your information and bill your insurance. If parents are not insured, the cost for a flu vaccine will be $40. There are no co-payments for flu vaccines.  To ensure adequate supply for our patients, we will not be offering flu vaccine for grandparents or other relatives as this time.

We use only the preservative free version of the flu vaccine.

Children 6 months to 8 years old may need a second dose of the flu vaccine 4 weeks after their first dose if:

  • This is their first time receiving a flu vaccine OR
  • They have only had 1 prior flu vaccine before this season.
