Insurance and Billing

Please contact our office to verify acceptance of your insurance plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure. We are pleased to note that as of August 2017 we are designated as Horizon Omnia Tier 1 providers

What To Expect

Being well prepared for your appointment will ensure that the doctor has all the necessary information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. We encourage you to come at least 15 minutes early to your appointment to make sure that all the paperwork is done. If required please have your referral, reason for the visit and results of any lab work, imaging studies or previous procedures available for review. We suggest that you have this information faxed over prior to your appointment or bring copies with you. Please also bring along an updated list of your medication. We look forward to your first visit.


Office Policies

Please arrive 15 minutes early for all appointments. We understand that occasionally emergencies occur that are beyond our control and either the doctor or the patient may be running late. Please realize that some patients may require additional time than allotted. If you arrive late, the doctors will try and accommodate you, however we try to respect everyone’s time and you may need to reschedule. Our office makes every effort to try and see our patients in a timely fashion.

We typically call to confirm an appointment 3 days prior to the scheduled date. If we leave a message and you cannot make the appointment, please call us to cancel so we can offer the appointment to another patient. Please make sure that our office has 2 contact phone numbers to avoid any charges.
