Membership 1: Mobile Acute Urgent Care
- 24/7/365 direct access to a board certified MD via phone call or text with guaranteed response time of less than 15 minutes
- Unlimited advice, triage, and text/telephone follow up
- Limited membership panel to allow physicians to provide personalized care without the time pressure of traditional model
- 1 telehealth or in home sick visit per child per year (additional @ $75/telehealth and $150/in home sick visit)
- Phone follow up within 24 hrs of all telehealth and in home sick visits to check on progress and address any additional questions/concerns
- Annual physicals, chronic health issues, and behavioral health not included in this membership (see primary care membership below)
Annual Membership Fee: $1200/yr - 1 child, $2200/yr - 2 children, $3000/yr - 3 children, $3600/yr - 4 or more children
Membership 2: Comprehensive Pediatric Well and Acute Care

- Limited membership panel to allow physicians to provide personalized care without the time pressure of traditional model
- Annual physical (in home) plus 2 telehealth or in home sick visits per child per year (additional @ $75/telehealth, $150/in home sick visit)
- Phone follow up within 24 hrs of all physicals, telehealth, and in home sick visits to check on progress and address any additional questions/concerns
- Comprehensive care including behavior and chronic health issues
Annual Membership Fee: $2400/yr - 1 child, $3600/yr - 2 children, $4800/yr - 3 children, $5600/yr - 4 or more children
Note: there will be an additional service fee for children under 2 to cover vaccine administration and increased frequency of well visits.
Service fee: $600 under 1 year of age. $300 between 1 and 2 years of age.
