Please print and complete the forms prior to your appointment and bring them with your insurance card. If you are unable to print the forms, please plan to arrive 10-15 minutes early to your appointment so that they can be completed in our office. Insurance Coverage: Most insurances accepted
Island Dentistry accepts many types of insurance policies. As a courtesy to our patients, we will file your claim on your behalf. It is imperative that we receive all of your insurance information accurately. Please review your insurance policy and be aware of your benefits. At the time of your visit, we will collect any estimated co-pays and/or deductibles.
Patients often believe that their insurance policy will cover most of their dental costs, but most only cover 50 to 80 percent of the average fee. We will help you estimate your share of the cost of treatment. However, we are not responsible for how your insurance company will handle each claim. Your insurance contract is between your employer and the insurance company. We are here to help you understand your insurance information and get the benefits of the insurance your employer chose for you.
Please keep us informed of any changes, such as the name on the policy, the address of the insurance company, or a change in employment. Rest assured that we will do everything possible to ensure that you receive the full benefits of your policy.

DOCPAY payment plans
DOCPAY payment plans are a simple and effective way to collect money from patients who cannot pay all at once. Our service allows you to automatically deduct money from your patient's bank account or credit card each month. Doc Pay is interest-free and requires no credit check. There is a convenience fee per payment.

At Island Dentistry, we believe everyone should have access to affordable dental care. We've designed an in-house VIP membership to give individuals and families without dental insurance an affordable solution.
