New Patients Welcome!
Your first visit to Goodall Family Dentistry establishes the vital foundation for us to partner with you in your dental needs. During your first visit, we will obtain important information, like your medical and dental history and collect intra-oral photos and radiographs. If you have x-rays from prior dental practices it is important that you have them sent to our office prior to your first appointment. Please visit the patient form area and print forms for your first visit. Remember to bring a list of your medications you are currently taking, photo ID and all dental insurance cards with you. Please arrive to your appointment at least 10 minutes before to complete any additional forms or answer any questions the front team may need from you.
