Planning for Dental Implant Surgery

Placement of Dental Implants starts with careful and detailed planning. X-rays and impressions are taken to plan the restoration and study the site of future dental implant/implants. Procedure for dental Implant  Surgery

During surgery for dental implants a preparation is done into the bone to place the implant titanium root. The amount of time required for dental implants to heal is generally between 2–6 months. If dental implants are loaded too soon, it is possible that the implant may move which results in failure. However, all-on-4 dental implants are often loaded immediately with a temporary denture.

There are different approaches to place dental implants after tooth extraction. The approaches are:

  1. Immediate post-extraction placement of dental implants.
  2. Delayed immediate post-extraction implant placement (2 weeks to 3 months after extraction).

If you do not have enough to hold dental implants, the site is first built with a bone graft.

Bone graft with dental implants is a safe and highly successful procedure that involves the “building up” or adding bone to the jaw by using your own natural bone from another location and/or by using donor, processed or synthetic bone materials. Bone grafts may also be used for sinus lift.

Dental Implants can be placed at the same time or after the grafted bone has fused or become a strong, integrated part of the existing bone. The amount of time the integration takes varies depending on the location of the graft and the density of the bone. It may take three or more months.

Platelet Rich Fibrin (PRF)

Sinus Lift

An expanded sinus may not allow the placement of dental implants in the back part of the upper jaw.

A sinus lift (sinus augmentation or sinus elevation), is a bone augmentation procedure for patients who have the insufficient natural bone in this area for placement of dental implants. The procedure involves adding a bone graft and/or platelet rich plasma or platelet rich fibrin below the sinus so that dental implants can be placed. Dental Implants can be placed at the same time or after the grafted bone is healed.  In some situations, sinus lift procedure can be avoided by placing short dental implants under the sinus. Types of sinus lift before dental implants:

Sinus lift can be done directly or indirectly. 

Direct sinus lift:

This is also called lateral window sinus lift. A window is cut into the bone of the outer wall of the sinus. The sinus membrane is elevated and a bone graft is packed. Direct sinus lift is indicated when there severe bone resorption at the sinus floor that does not allow placement of dental implants.

Indirect sinus lift:

This is also called crestal sinus lift. This procedure is far less invasive than the direct sinus lift. The sinus in entered through the ridge or through an extraction socket. A bone graft is then placed through the socket.

Ridge Expansion 

If the jaw ridge isn’t wide enough to support dental implants, the bone ridge is expanded so that a bone graft and platelet rich plasma can be added. Ridge expansion is an excellent way to place dental implants in areas of thin bone and to avoid more invasive procedures requiring extensive bone grafting. In some situations, implants can be placed right after a ridge expansion.
