Your First Visit
At Smile Forever Family Dentistry, you will experience unmatched customer service from the first moment you enter our facility. If you have expressed any amount of fear or anxiety prior to your visit, we will take time to reassure you and answer all of your questions. Our time is reserved for you, and you are our top priority.
To streamline your initial visit, we ask that you arrive on time with your referral slip and any applicable x-rays, a list of medications you are presently taking, and your driver’s license and insurance card in hand so that you can fill out patient-registration forms.
During a standard new-patient visit at Smile Forever Family Dentistry, you first meet with the hygienist for a medical and dental history review, x-rays, and intraoral photos. Additional diagnostic procedures you can expect include a periodontal screening, a comprehensive oral evaluation, an oral cancer screening, and a TMJ palpation. In the absence of periodontal disease, a teeth cleaning will be performed, and we will offer preventive services such as fluoride treatment and sealants. , walking you through our findings. We'll work closely with you to discover a dental plan designed to meet your unique oral health goals. If you need to reschedule your appointment, we encourage you to do so 48 hours prior to your scheduled appointment; cancellations need to be made 24 hours in advance to avoid a $50 fee.
