What to expect
Being well-prepared for your first appointment will ensure that the office has all of the needed information to provide the best possible care for you. It will also help relieve any unnecessary anxiety you may be feeling. Your first visit to The Foot & Ankle Center, PC establishes a vital foundation for our relationship with you. During this visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Talarico. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the new patient forms in advance of your appointment. What to bring with you to your appointment:

  • Insurance card (please make sure your card is current)
  • Referral, if required by your insurance plan
  • Driver’s license or other photo identification
  • Name of your Family / Primary care Doctor and the date of your last visit
  • A list of all current medications and allergies
  • If you are seeking a second opinion or transferring your care, your most recent medical records and/or x-rays

When to arrive for your appointment:
We make every effort to keep on schedule. A patient being seen for the first time may require 45 – 60 minutes for their initial evaluation. Therefore, we ask that you please arrive 15 minutes prior to your scheduled appointment so that the required information forms can be completed and your appointment can begin on-time.

New Patient Information Forms:
To save some time, you may choose to print the forms, fill them out at home and bring them with you on your first visit.

Cancellations:
If you are unable to keep your appointment time as scheduled, please allow a minimum 24 hour courtesy for cancellation so that we may fill that time spot with another patient.
