We require a 25% deposit of your treatment planned appointments. This deposit goes toward the cost of your treatment upon completion.
The office requires at least 48 hours notice to cancel an appointment in order to avoid the late cancellation fee of $50.00 per 1/2 hour up to the cost of treatment.
