If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Payment of Co-Pay is expected at the time of services rendered. Depending on the procedure payment plans may be available. We do our best to estimate proper co-pays, however in the event that additional amount is owed, or a claim is denied by your insurance a statement will be sent in the mail.
Dental Insurance
We accept most PPO insurance plans. Insurance eligibility verified based on the information provided by the patient. Information is taken upon making the first appointment. Insurance verification is a quote of benefits not a guarantee of payment.
