Appointments

Insurance, Billing and Payment Options
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We accept all traditional insurance plans. You may contact our office to verify your plan. We are happy to accept your insurance for your reimbursement as long as you are free to choose your own dentist. Dr. Blavin does not participate in HMO's (but we will be happy to file your HMO claims for you).
