Foot Clinic APPOINTMENT S

Please arrive to your appointment at the correct time so we can spend the necessary time with you. If you require a foot soak before treatment, you need to arrive 15 minutes before your treatment time.  If for any reason you are unable to keep your appointment, please contact us ASAP as we have a waiting list of patients who could use this appointment time.  THANK-YOU.

FEES AND INSURANCE

We will be pleased to discuss fees and payment arrangements with our patients and their relatives.

PATIENTS ARE DIRECTLY RESPONSIBLE TO OUR OFFICE FOR THE TOTAL COST OF TREATMENT

1. We will provide a completed insurance form, as well as all other required documentation for orthotics with fees to be submitted to your insurance company for reimbursement based on your benefits.

2. We follow ethical insurance guidelines of dealing directly with our patients for fees.  Your insurance benefits are paid to you in most cases.

3. Your insurance reimbursements may be less than our office fees because most insurance companies:

A. Do not base their coverage on the current years podiatry / chiropody fee guide.

B. Do not provide any coverage for some services.

OHIP DOES NOT COVER ANY OFFICE PROCEDURES FOR CHIROPODY OR ORTHOTICS

Ontario Podiatrist who were in practice before 1991 are grandfathered and allowed to bill OHIP.  All other Ontario Podiatrists and Ontario Chiropodists are not covered by OHIP. OHIP pays a very small amount of the total visit for those who are covered, but there is in many cases a disadvantage for those who have partial OHIP coverage as their Extended Health Insurance will not provide coverage while OHIP pays.  This is usually a disadvantage for the patient.  You should always check with your Extended Health Insurance provider to see if you will be reimbursed if your Podiatrist excepts partial payment from OHIP.  We know this fact only too well and the best thing that happened to my practice was the day that we lost the right to bill OHIP, although Ontario Podiatrists are not aware of this fact yet.

MANY PATIENTS WILL BE DENIED ACCESS TO THEIR EXTENDED HEALTH BENEFITS WHEN CONSULTING AN ONTARIO PODIATRIST.  WHEN YOU CONSULT AN ONTARIO CHIROPODIST YOU WILL HAVE FULL ACCESS TO YOUR EXTENDED HEALTH BENEFITS FROM DAY ONE, ALWAYS.

PAYMENTS ARE DUE WHEN SERVICES ARE RENDERED.  PAYMENT ARRANGEMENTS WILL BE CONFIRMED AT YOUR FIRST VISIT.

INSURANCE

Our foot clinic is covered by WCIB, DVA, ODSP and motor vehicle insurance. In the case of WSIB and DVA we have the patient pay upfront for the service until we receive written confirmation from the agency that we will be reimbursed.  Upon reimbursement, we refund the patient any fees that they have paid to our foot clinic.  Many employers extended health insurance plans such as Green Shield, Great West Life, Manulife, Sunlife, etc. cover some or all of our services which in the majority of times is paid directly to the patient after they have paid our foot clinic for our services.  Always contact your extended health insurance to confirm coverage or your human resources department of your employer.  The patient is always responsible for their account, although many insurance companies provide some or total coverage for most of our services.  We always assist the patient with their insurance paperwork.  Please call our office to verify acceptance of your insurance carrier.  Please note that the patient usually has to be present for insurance verification during the hours that the insurance company is open when we call.  Our office is very familiar with many companies insurance, especially government and large companies.

UNDERSTANDING EXTENDED HEALTH INSURANCE

EXTENDED HEALTH "INSURANCE"  is a pooled disbursement of your (or your employer's) money.  The insurance company, a for-profit business, controls this money.

The perception is "they will pay for everything" and someone wants "something for nothing."

However, it does not work that way.  As a business, the insurance companies have costs and are required by their shareholders to make a profit. If for example, administrative costs and profit were calculated to be 30%, where is the money going to come from?  Well, it has to come from you as a patient and the provider in some way.  As long as you recognize this that the pooled money is just that and you may not get all your money back, they are not going to pay for everything.  Also payment may be delayed due to administration and then you may understand the reality of insurance.

In some cases my Foot Clinic may be able to provide predetermination for you to determine your particular plans benefits level of reimbursement for you to determine your particular plans benefit level of reimbursement for you (of your money).  A reasonable reimbursement expectation is payment of 60% - 80%.  Our fees are based on the standard fee schedule published by the Ontario Society of Chiropodists.

Our biggest concern is that there will be no interference with our foot specialist-patient relationship.  Our patients recognize the value and benefit of needed podiatry procedures and custom orthotics, which is our responsibility to explain to you and perform responsibly.

We have staff which will provide excellent service to help you determine your benefits.  Since our responsibility is to you and your responsibility to us is that we have a clear, direct foot specialist-patient relationship; therefore you are responsible for payments to us and we are responsible for podiatric care to you.  Your insurance company, which holds your 'benefits package', is responsible for reimbursing you dependant on your particular individual plan with your employer.  If there are any questions we will try to assist you, but the final determination is based on your own particular benefits plan package. Please be aware that we can only confirm your benefits if you are physically present and the insurance company is open.  After hours or on weekends we will not be able to confirm your coverage and the patient is responsible to ensure that they are covered.
