New Patients

During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.  We accept most insurances.

Patients visiting Roadrunner Foot and Ankle are advised to arrive with their toenails devoid of any nail polish, including gel nail polish. This precaution is not merely a matter of preference; it's a crucial step in ensuring the effectiveness and accuracy of their appointment.

In the event that a patient arrives with nail polish, the clinic may need to reschedule their appointment. This is not done to inconvenience the patient but rather to uphold the standards of care and safety. Rescheduling ensures that the patient receives the full attention and quality of service they deserve without compromising on safety or efficacy.


Patient Forms

Please print and fill out the New Patient forms so we can expedite your first visit.

Please arrive 30 minutes early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient's insurance card
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records
