• Membership plan only provides discounts at Karim Banani DDS.
• Member is responsible for costs of dental services but will receive a discount for those services as a member of this plan.
• Certain specialty procedures may not be within the scope of the practice, in such cases patients may be referred to a specialist whose services are not covered under this plan.
• Membership fees are non-refundable and non-transferable.
• Plan Term is 1-year from the date of purchase
• This plan does not cover Sleep Apnea treatment, dental products, sedation or cosmetic procedures.
• Plan benefits cannot be combined with dental insurance.
