New Patient Forms

Please download these, complete them and BRING THEM WITH YOU to your appointment!! We want most of your appointment time to be with the physician, not doing paperwork!!


Established Patient

Thank you for continuing to allow us to participate in your medical care. Contact us via your secure patient portal or by phone (732) 577-1999. Please be sure to advise us of any change in your medical condition, address and insurance coverage. If it has been three years or more since your last visit, PLEASE download and complete the forms here, and BRING THEM WITH YOU!!


Appointments

Appointments are of several types. A new patient office visit typically takes 30-45 minutes, so please allow sufficient time. Follow-up visits generally require 15-20 minutes. A NEW PATIENT is asked to arrive 20 minutes early to allow for registration and verification of insurance. ALSO, the Doctors need a thorough medical history about you, so we provide a Health Information form which you should fill out COMPLETELY in time for your visit. This saves you time and allows the Doctors to be more thorough. During a consultation, the medical history is reviewed and a physical exam is done. The detail of these varies with the complexity of the problem. We do not generally do "routine physicals" or provide primary care, but we are happy to refer you to a primary care physician if you need one.

We do our best to stay on schedule, although emergencies and delays sometimes arise. If we are seriously delayed, we will try to notify you beforehand. Please assist us by BEING ON TIME for your appointments. Infrequently, patients may be taken out of turn, in an emergency; we ask your understanding and patience.

Your Appointment

On your first visit, an electronic medical chart and billing record are established. BRING WITH YOU YOUR COMPLETED Patient Forms, YOUR INSURANCE CARD(s), YOUR DRIVER'S LICENSE, any referrals or authorizations required by your health care plan, recent pertinent radiology reports, EKG report and recent labs. PLEASE NOTE: If your health care plan requires a referral, we cannot see you without this documentation. 

After your exam, we may ask your permission to transfer some of your medical records from other doctors.


Cancelling an Appointment

If you are unable to keep your appointment or are going to be late, please call our office (732-577-1999) as soon as possible. This courtesy allows us to be of service to other patients. CANCELLATIONS WITHOUT ANY NOTICE ARE SUBJECT TO A CHARGE.


Insurance and Billing

When you contact us we verify whether we are part of your insurance plan network, or will advise you otherwise. For scheduled procedures our Billing Department's team will contact your plan for pre-authorization, but the final decisions about whether a claim gets paid is up to your health plan. Whether you see us in-network or out of network, we will file your insurance claim forms for you. As always, check with your plan about any aspect of your coverage you're not sure about.

Payment Options

Copayments are due at the time of your office visit. We will bill your insurance company for services rendered and  we will balance  bill you after your insurance company has processed your claim.

We send monthly statements of personal balances due. You should appeal to your insurance plan if you think the plan has underpaid for the services or if you are unexpectedly denied coverage. You should receive a notice from your plan about how they covered the claim.

We accept personal checks, MasterCard, VISA and Debit cards.

Prescription Refills

Please notify us of your preferred pharmacy. Most prescriptions will be sent electronically to your pharmacy, so if we have the correct information, refills can be efficiently processed. . Please use the Patient Portal to request a prescription refill or call the office for prescription refills during regular office hours. We cannot call in prescription refills during nights and weekends. Prescriptions will usually be sent to your pharmacy within 24 hours. Please allow time for processing and contact your pharmacy to verify that a prescription has been renewed before you proceed to the pharmacy.  Please contact us if the prescription has not been renewed in a timely fashion.
