contracts. Patients with a Primary Care Physician (PCP), Point of Service
(POS), or HMO option must have a referral from your primary care physician.
PPO enrollees do not need a referral but should pre-certify most
hospital/out patient facility based surgeries or testing.

Patients are responsible for obtaining all referrals as mandated by your
insurance plan. This office is not liable for fees not covered by your
insurance. All balances not covered by insurance are the patient's
responsibility. Check your provider directory if you do not see your plan
listed.
*PHCS is the managed care organization for several companies including the
Guardian, Fortis, and Great West etc. If your insurance card has the PHCS
emblem, we are a preferred provider for your plan.
