Use our online form or call 715.847.2558 or toll free 877.442.7762 (877.GIASSOC) weekdays between 8:00 am and 5:00 pm. Please have your insurance cards out and available when you call, we will need you to review them with us before we can make the appointment.

Office Visits
Patients are asked to arrive 15 minutes before the scheduled appointment time.  Please be sure to bring your  current insurance cards, prescription and over-the-counter medications and any supplements you are taking to your appointment. A parent or guardian should accompany patients under the age of 18. Allow a minimum of one hour for regular office visits. Out of courtesy for others, patients arriving late may be asked to reschedule.

Appointment Cancellations
If you are unable to keep a scheduled appointment, please let us know at least 24 hours in advance so we can offer that appointment time to other patients. Repeated failure to present for scheduled appointments may result in discontinuation of  services.
