Please have your insurance cards out and available as we will review them with you before we can make the appointment. To request an appointment, fill out the form below.
Office Visits
Patients are asked to arrive 15 minutes before the scheduled appointment time. Allow a minimum of one hour for regular office visits. Please be sure to bring your current insurance cards, prescription and over-the-counter medications and any supplements you are taking to your appointment. Out of courtesy for others, patients arriving late may be asked to reschedule.
Appointment Cancellations
If you are unable to keep a scheduled appointment, please let us know at least 24 hours in advance so we can offer that appointment time to other patients. Repeated failure to present for scheduled appointments may result in discontinuation of services.
