Appointments

We offer well child and same day appointments. Please call to schedule. If you have an appointment that you will not be able to keep, please call ahead and cancel 24 hours in advance or there could be a fee assessed. We can then use that appointment time to assist someone else in need.

Insurance and Billing

We require your insurance information in advance so that we may verify coverage and establish if any fees are due at the time of visit. As of July 1, 2017 all Co-Pays/Deductibles/Co-Insurance will be collected at the time of the visit.  Please make sure you check with your benefits prior to your appointment times so that you are prepared to pay amount due. We accept M/C, Visa, Discover, cash, check, HSA cards and/or HSA checks. If you have questions regarding your amount due, please call our Office Manager at 480-792-1012.

Payment Options

We accept checks, cash, M/C, Visa, Discover, AmEX, HSA cards and HSA checks. All Co-Pays/Deductibles/Co-insurance and any past due balances are due at the time of visit.
