To verify that we are in network with your individual group plan, please contact your insurance company directly.

Payment Options

HMO, POS, and Aetna patients must be referred by their primary physicians. Since insurance plans differ, we ask you to be aware of what your plan covers and of your responsibilities in regard to co-payments and deductibles.  Co-payments and/or deductibles must be paid at the time of office and ambulatory surgery visits.

Please provide the office with your current insurance cards and notify us if there are any changes in your insurance plan.

We will file your insurance claim for you.  You will receive a bill for services after the insurance company has paid its share of the charges.
