THE SMILE CLUB
Join our Smile Dental Club. Our goal is to make everyone SMILE! The Dental Membership Club has been created for our patients who do not currently have dental insurance. For a low annual fee your membership will include the following services:
- Two Complete Oral Examinations
- Two Healthy Cleanings (free of previous periodontal or gum disease)
- Full Mouth Digital X - rays
- Oral Cancer Screening
- Two Fluoride Treatments
- 15% courtesy on all additional dental treatment at our office

Appointment Scheduling Details
Patients will be notified of appointment request within 24 hours by our Appointment Coordinator.
Do You Have Dental Insurance?
We accept most fee-for-service dental plans. Even though dental insurance is a private arrangement between you and your company, we will file your claim, handle all the paperwork and help you get the maximum insurance benefits. Also ask about our financing options.

For your convenience, we accept personal check or cash payments, credit card payments (Visa, MasterCard, Discover, American Express), or your personal financing through Care Credit. Please note that an additional charge of $35.00 will be added to your bill for each returned check.
If you have questions regarding your account, please contact us. Many times, a simple telephone call may clear any misunderstanding.
We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We will be happy to assist you in making arrangements for a monthly payment plan through an independent lending institution, but that mutual agreement among all parties must be reached prior to the actual procedure.
