What To Expect
Being well-prepared for your appointment will ensure that your healthcare provider has all of the needed information to provide the best possible care for you. It will also help alleviate any unnecessary anxiety you may be feeling prior to your first appointment. Please bring your insurance cards and co-pay (if applicable), photo ID (driver's license), current medication list and any pertinent medical records. We look forward to your first visit.
Treatment of Minors
A parent or legal guardian (over the age of 18), must accompany minor children (under the age of 18) to the inital office visit to sign all paperwork. If the minor will attend future visit unaccompanied, a "Consent To Treat A Minor Form" must be on file with the office. These forms can be obtained from the Front Desk or found on our website under the Patient Forms section above. The form is valid for 12 months and must be signed annually, when signatures on other practice forms are updated.
Price Transparency
Senate Bill 17-065, or “The Transparency in Health Care Prices Act,” helps to ensure “self-pay” prices are accessible to the public by requiring health care providers to post their 15 most common procedures.
If you are covered by health insurance, you are strongly encouraged to consult with your health insurer to determine accurate information about your financial responsibility for a particular health care service provided at Hill Center for Dermatology, PC. If you are not covered by health insurance, you are strongly encouraged to contact our billing office at 303-526-1117 , option 3, to discuss payment options prior to receiving health care services from a health care provider at our office, since posted health care prices may not reflect the actual amount of your financial responsibility.
The prices listed below are for the standard service for a particular diagnosis and do not include any amount charged for complications or exceptional treatment. The actual charges for a visit are dependent on the circumstances at the time service is rendered.
