How to Send Us Your Handpieces

Below are two items you will need to print out to get your handpiece to us. You may click on each item to open them off the site as a PDF, or you can "Right Click" and “Save As" to save them to your computer for printing. Both PDF's below are "fillable," meaning you can use your keyboard and mouse to fill in the information.

UPS Return Service Pre-Paid Label-Preferred Shipping Method

Please provide us with the following contact information:

  1. The name of your contact person.
  2. Your office name and mailing address.
  3. Your telephone numbers. Please be sure to provide a secondary telephone number.

If you would like us to fax the UPS label to you, please include your fax number in addition to the information we need from you in numbers 1 -3, above.

If you would like us to email the UPS label to you, please include your email address in addition to the information we need from you in numbers 1 - 3, above.

US Mail - Postage Paid Mailing Label

Cut this label out and tape it securely to any sturdy box you have in the office. Just give this to your US Postal Carrier and we should receive your handpieces the next day.
