Your first visit to David R. Leonoff, DDS, PC involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can also save some time by printing out and completing the patient forms in advance of your appointment.

If you have a scheduled appointment for a consultation with Dr. Leonoff, you should bring the following items to your first visit:

Referral card from your dentist - so we know what you and your dentist need from us.

X-rays - without complete x-rays we cannot make an accurate diagnosis.  For limited problems, an x-ray of the area is usually enough.  For a comprehensive exam, we need a Full Mouth Series or a Panorex.  If necessary, we can take any needed x-rays.

Insurance information - if you have insurance, please bring your proof of insurance (ID card).  We need to know the insurance carrier, group number, name and information of the insured as well as the patient (if different, such as spouses).  Click here to see what insurance we accept.
