The Initial Visit
Our compassionate Spring, Tomball, and Willis pediatricians do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and your child are well-prepared for your first appointment. Our Mission
At Champion Pediatric Associates, our vision is to provide compassionate, comprehensive medical care to children of all ages (birth to 19 yrs.) while maintaining a warm, friendly, service-oriented environment for the whole family
Patient Intake Process
To make the new patient intake and check in process more efficient for our patients we have partnered with Phreesia. Phreesia allows our families to keep up to date with all office policies, update your child’s demographics, insurance cards and consents. Phreesia will text and or email Up to 3 days prior to your scheduled appointment with a reminder of your child’s appointment time and location followed with a link to complete our new patient forms and policies in the comfort of your own home. Following all appointments after your initial visit you will continue to receive text reminders of your upcoming appointment and a precheck link that will need to be completed prior to your appointment time.
What to expect
A pleasant, comfortable first visit builds trust and helps put the child at ease during future visits. We want your child to enjoy getting to know our doctors and staff, so we work hard to establish this bond during every appointment to our office. During your initial appointment, we will explain everything in detail and answer any questions you may have.
What to bring
Please bring a current Insurance card and Photo ID. We also ask that you bring your child’s Immunization records at your initial visit to prevent any delays in receiving your child’s routine vaccines.
