Welcome to Streamwood Smiles! We would like to thank you for allowing us the privilege of participating in your dental health. Great dental health is our goal for every patient.
Your first visit involves a few special steps so that we can get to know you. Please find important patient information forms below to assist us with your registration. You can save some time by printing out and completing the patient forms in advance of your appointment. If you have dental insurance, please bring your insurance identification card to your first appointment; at which time we will make a copy to be kept in your patient chart.
We serve the greater Streamwood, IL area including....Bartlett IL, Elgin IL, Hanover Park IL, Hoffman Estates IL, Schaumburg IL, and South Elgin IL. Our office is conveniently located in Streamwood, near the corner of Sutton Road (Route 59) and Schaumburg Road. See driving directions below.
