Planning for your first appoint - here's how:
Your first visit will last approximately 30-60 minutes.

If you had x-rays taken at your dentist’s office, we may be able to use them. If not, it is likely we will need to take any necessary to aid in your care. We prefer periapical type films generally, although others may be needed depending on the condition.

During your first visit we will discuss with you your health history and your periodontal condition. We will complete an examination and discuss any findings. An additional appointment may be needed to develop a thorough care plan. We encourage you to bring a spouse, significant other or trusted friend to attend the care plan appointment with you.

Your dentist will be notified as to what is needed once a care plan has been agreed upon; and a detailed letter will be sent to your dentist outlining our findings and the recommended care needed.

What to bring to your first appointment

  • The medical history form filled out
  • The HIPPA form filled out
  • Your insurance card (if applicable)

All forms are mailed to you when the initial appointment is made or they can be downloaded

We want your time with us to go smoothly. So please understand that if you have had a dental examination within the last 6 months, your insurance company will not cover your first visit with us. We ask that payment be made for this visit at the time of the visit. It will be submitted to your insurance company and if it is paid, you will be reimbursed.

Cancellation Policy
If an appointment needs to be cancelled or rescheduled, we ask that you give us 24 hours notice.

What To Expect
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling. Educate yourself on your symptoms by reviewing the content on this Web site. Also, take some time to review our staff page and familiarize yourself with our staff. We look forward to your first visit.


Insurance
As a courtesy to our patients, we will submit your insurance claims to your insurance company.

Billing
Unless prior arrangements are made, payment is expected at the time of service. We do offer a 3rd party plan,  through Car Credit which our patients have been pleased to use. They offer quick and easy financing for up to one year.

Financing
There is a fee for the first appointment, and for a second appointment if needed, to review your care plan.  The cost of the second appointment can be applied to the cost of future care, although we may not participate in your dental plan(s), many plans will cover some of the cost for “out of network” specialty care.
