New Patient Forms

After making an appointment we will provide you access to your Patient Portal where you will be able to view and update all your medical records.

*Minors must be accompanied by their parent or guardian at their New Patient Appointment at Moore Foot & Ankle Specialists, PA. At this time, the minor's parent or guardian will be able to complete an Authorization to Consent to Health Care for Minor Form for future appointments for someone else to bring the minor patient to their appointments.
