You may enroll in this program at any time. Payment for the program is to be paid in full at the time of purchase. The plan runs for one (1) year from inception date. No refunds or credits are given at any time. This offer cannot be combined with any additional discounts. Please write down the appointment dates and time that you will schedule.
PREVENTION PACKAGE TERMS AND CONDITIONS:
We are offering this plan to help our patients maintain and achieve good oral health. In order to be fair to all patients and employees the following conditions exist:
The 10% discount will be applied to all restorative treatment on the day services are rendered. Payments are expected in full on the date services are provided, unless a payment arrangement has been agreed upon.
When changing your hygiene appointment with more than 48 hours notice, we will gladly reschedule your appointment. Any missed or forgotten appointments less than 48 hour cancellation policy will count toward your hygiene appointments in your package. Additional hygiene appointments are charged at our regular office fee.
Each package is good for only one (1) person and is not transferrable or refundable.
