No, you may download all new patient forms from this page and complete at your convenience prior to appointment day.

Patients of Record

My child is sick. How quickly can I get an appointment?

We make every attempt to offer same day sick visits. Please call us at 281-265-2272.

I’m concerned about my child and it’s after hours or on a weekend. What do I do?

As a patient of record, you have access to our on-call physician. Please call us and our on-call physician will be contacted.

How can I access medical records or request appointments and prescription refills?

Our patient portal is perfect for that. Please contact us to receive a username & password to begin using this wonderful system!


Insurance

Do you accept my insurance?

Please see the above list of insurance plans we accept. Since we regularly expand this list, please call us if you don’t see your insurance.

When is my co-pay due?

Your co-pay is due at the time of your appointment. We thank you for coming prepared to make this payment.

Do you file my insurance?

Yes. As a courtesy to our patients, we will file your insurance on your behalf.

I have a question regarding my insurance policy or reimbursement. What do I do?

Please contact your insurance’s customer service department. They are in the best position to assist you.


General

What is the best way to keep my child healthy?

Our physicians believe that illness can best be prevented through regular well check-ups and immunizations, as prescribed by the American Academy of Pediatrics.

My child has a chronic illness or special need. Can they be treated at your practice?

Absolutely. As physicians specialized in pediatrics, our doctors are fully trained to treat a wide variety of issues and also children with special needs.

Aside from insurance, how else can I pay for my child’s visit?

For your convenience, we also accept cash, checks and credit cards.
