Thank you for choosing to schedule an appointment with Dr. Anna Natcher.
We will call you approximately 2 days PRIOR to your appointment to remind you of the date and time. If we have to leave you a voicemail, you must call us back to confirm your appointment at least 24 hours PRIOR to your appointment in order to keep it. We have a Waiting List of patients that are in pain and need to be seen as soon as possible. We are also a small office and in order to continue to treat patients, we cannot afford NO SHOWS or last minute cancellations. If calling us back to confirm your appointment is too much of an inconvenience for you, you may absolutely email us at [email protected] 24 hours PRIOR to your appointment to confirm.
We have included a New Patient Information Packet (click below) for you to print, fill out, and bring with you. Or you may arrive 20 minutes PRIOR to your appointment time to get them and fill them out here at our office. We recommend that you leave plenty of time to find our location. We are in the same building as Signature Smiles dental office and our door is located to the SOUTHWEST part of the building (if you are looking at our building from the parking lot, we are to the LEFT and back of the building).
Please bring your insurance card with a picture ID with you. If you have an HMO plan, we need a referral from your doctor PRIOR to scheduling your appointment.
