             For your first visit, please be sure to arrive 15 minutes before your appointment time to fill out Patient Form and bring the 

following information with you:

• Your Dental Insurance Card.
  
  
• Driver License.

Payment options: 

                        We accept Check, Cash, most major credit cards (Visa, MasterCard, American Express, Discover)

* We do not accept Payment Plans. Estimated Co-Pay will be due at the same date of your visit. We will help to send your visit claim to your insurance as Courtesy. Patient is responsible for any unpaid or rejected claims. Please contact your insurance directly regarding your dental plan. 

Insurance: 

- We ACCEPT PPO & Medicare Advantage Plans.

- We DO NOT accept Medicaid - Medicare- or HMO plans.
