Appointments
Initial Consultation Visit: Your first appointment is usually a consultation. The doctor will review your medical history and do an examination. If you have x-rays from your dentist or physician, please bring the x-rays with you. When necessary, we will take additional x-rays.
- The referral form or letter and any x-rays from your dentist
- A list of medications you are taking
- Insurance cards and information
For your convenience, you can print out and complete the necessary registration forms ahead of time. This will expedite your registration process. You will find the forms in the New Patient Section.
All patients under age 18 must be accompanied by a parent or legal guardian.
Patients who plan to have IV anesthesia for surgery must have a consultation visit.
When you make your appointment, inform our office if you have a medical condition, for example: diabetes, high blood pressure, a heart condition, artificial heart valve, or artificial joint. Alert our staff to any recent hospitalization or serious illness. Also, please notify us if you take heart medications, aspirin or anticoagulants.
Please notify our office if you have recently traveled out of the country.
Insurance Information
We accept or participate with many medical and dental insurance plans. Please contact our office for more information. We will work with your insurance company on your behalf to maximize your insurance reimbursement for covered procedures.
Payment Options
Our office accepts Visa, MasterCard, Discover, American Express and CareCredit. We deliver excellent care at a reasonable cost to our patients. Therefore, payment is due at the time of service, unless other arrangements have been made in advance.
Please remember you are responsible for all fees charged by our office for your treatment, regardless of your insurance coverage.
Facilities and Equipment
We take pride in our comfortable and modern oral and maxillofacial surgery office. Our facility is equipped with state-of-the-art technology including low-dose digital oral and panoramic dental x-rays, modern oral surgery equipment and anesthesia monitors. Our office and surgeons are credentialed by the State of Maryland and the American Association of Oral and Maxillofacial Surgeons (AAOMS) to administer all forms of sedation from inhalation analgesia to intravenous sedation and general anesthesia.
