Insurance

DDC physicians are providers for most major insurances and Medicare. Contact your insurance provider or call our office for more information.

We will automatically file your primary and secondary insurance for our services. You will be asked to verify your insurance at each visit. You are also responsible to pay your copay at each visit.

Please remember that although we file your insurance, you remain responsible for any balance on your account. You may make arrangements with our patient accounts representatives for monthly payments if you are unable to pay the balance of your account within 90 days.

• Medicare - We accept Medicare assignment. You will be required to pay the remaining 20 percent of Medicare’s approved amount either by supplementary insurance or personal payment.
• PPO and HMO - If you see a participating provider, all copays are due at the time you register at the reception area.

Hospital Affiliations

Our DDC physicians are on staff at: St. Mary’s Hospital, Riverside Medical Center, Silver Cross Hospital and, Iroquois Memorial Hospital. Please check with your insurance company to see which hospitals are within your network.
