If you or a family member are insured by United Health Care Community Plan, please be aware, UHC has changed their referral policies. Your primary care provider must submit an electronic referral via the UHC website. As per UHC, referrals on standard prescription pads are no longer accepted.

If a referral has not been received, we will do our best to inform our patients. We are sorry for any inconveniences this may cause. This is new to everyone!
