Preparing for your visit and what to expect

To fill out paperwork securely online, please click here. Or to print out your paperwork to bring with you to your appointment, click New Patient Forms.

Arrive 15 minutes early to your appointment so we can complete your integration into our practice. Also, bring the following:

  • Insurance Card or Cards
  • If multiple insurance, please indicate which is primary and secondary
  • Photo ID
  • List of prescriptions and/or over the counter medications including does and frequency
  • Recent test results, x-rays, or relevant records
  • Referral from your primary care physician if needed
  • The shoes you wear the most
  • Method of payment
  • Please read: HIPAA

You will be greeted by our staff in a warm and welcoming environment to check in. During your first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Blakeslee and our team.

Insurance and Payment Information

You are responsible for co-payments, deductibles or charges that are not covered by your insurance. If you have questions regarding your billing or which insurance plans we accept, please call (732) 833-6888. We have partnered with Cleargage and Dr.Chrono for paperless statements and another avenue of support for billing questions. Questions regarding insurance coverage and benefits should be directed towards your employer or insurance company.

Account payments can be made at the time of check-in or at any time in person at our Jackson location. Accrued payments can be made by mail, phone or your Cleargage log in. There will be a $30.00 returned-check fee assessed when applicable.

After your visit, you will receive a link for your patient portal. Balances can be made through your account. Call the office for more details.

Current Patients

Do you need to update your information before your next visit with us? Often patients ask why do we ask for your medical history yearly. This allows us to maintain accurate records about your health and allows Dr. Blakeslee to give you the best possible care. This information gives important clues about what is going on in your health and tells Dr. Blakeslee what you may be at risk for in the future.
