What to expect at your first dental appointment with us:
- A warm welcome as you arrive
- Dental and medical history reviewed
- Comprehensive dental exam from Dr. Reid
- Digital x-rays as needed
- Discuss the health of your teeth and gums, and make a personalized plan for your dental health
- Answer any questions or concerns you have about your smile
Patient Forms
Insurance and Payment Options
Hilliard Dental Associates is an in-network provider with many dental insurance plans and can accept any PPO insurance plan that allows you to choose your provider. We recommend you contact your insurance provider to confirm coverage details. We are happy to submit claims for you after your dental visit. For your convenience, we accept cash, checks, and major credit cards. We also offer financing through Cherry and Care Credit.
Please note: we are unable to accept Buckeye, Medicaid, Molina or CareSource insurance plans, or any HMO/DMO insurance where you are assigned to a specific provider.
