PATIENT PORTAL

For new patients, once you (1) register online, wait for your confirmation email.  Here you will be asked to verify your date of birth. And then complete a short questionnaire.  The link you need is in this Welcome Email. You may need to RESET your password.  That's okay!  Once you verify your account, you will be asked to login (3), with your email and password. Then one final verification process.  Check you spam or junk mail for communication.

Once in the portal, find your Dashboard to make appointments, message Dr. Mechanik, and access your medical records.  Please click (4) EDIT MY INFORMATION to enter your address and insurance information. And you have some homework: (5) You will receive a link, by text and email, asking you to check-in for your appointment and complete our Patient Registration.

EMAIL ADDRESS

The system will verify  your email address by sending you a verification email.  Please make sure you have access to that email account entered.  If you have a misspelling, do not worry.  Just let us know and we can update your records.

INSURANCE

We are members of most, but not all insurance plans. And we are out-of-network with the following, Bright, Friday, Humana, Kaiser Permanente, and Medicaid. We are happy to accept out-of-network benefits' usual and customary charges. Please contact your insurance company if you have any questions concerning your benefits.
