Please give yourself extra time because there is extensive road construction at Steeles. Ave E. between Kennedy Rd. and Midland Ave. Our clinic address is 4675 Steeles Ave. East, Unit 1C25, Scarborough, ON. M1V 4S5. Our building is named Splendid China Mall. The major intersection is Kennedy Road & Steeles Ave. East, near the south-east corner.
By Car : Drive along Steeles Ave. East. If you are coming from the east, turn left (south) into Redlea Ave at the traffic light (it is the 2nd traffic light past Midland Ave), and then turn left into the driveway with the sign "Milliken Go" . If you are coming from the west, go past Kennedy Rd, and turn right at the next traffic light onto Redlea Ave, and then turn left into the driveway with the sign "Milliken Go" .
Exiting by car: Must go south from the mall and turn right into Redlea Ave. which will lead you to the traffic light at Steeles Ave. E.
By TTC Bus : Get off the bus stop at Steeles Ave. E. & Kennedy Rd. intersection and walk about 8 minutes.
By Go Train : Get off at the Milliken Go station. We are just north to the station.

Appointments and Patient Intake/Update Forms
As Ms. Cindy Chow is a licensed chiropodist (foot specialist of Ontario), it is required that any person who wishes to be a patient / client of hers that an intake form and a patient contract / consent form is to be completed. Please note that the intake forms should be read and filled out prior to your initial visit . We would be happy to email you the forms or you may pick the forms up at our office before then. Also, you may fill out the paperwork on the day of your visit so long that it is completed before your scheduled appointment time; so we suggest you arrive 10-15 minutes early for this.
Please call to book your appointment. Each of our clients are seen by scheduled / booked appointments. Our receptionist will call to remind you of your appointment time in 1 or 2 clinic working days before. You may email us through our website's contact form a request for your appointment and we will reply within our next work day. Please provide your telephone number and name when using the contact form. Kindly give us 24 hours notice for any cancellation, changes to time, or for being late (late by missing half or more of your appointment) to avoid a charge on your next visit.
Billing / Payments / Insurance
Payment is due on the day services are rendered. Payment for products such as orthotics are due on the day they are dispensed. We require a deposit payment for orthotics and shoe orders (half of the total). We accept cash, E-transfer, debit (Interac), Visa, MasterCard and Amex credit cards. We will provide an official receipt for the services rendered and/or purchased items. Our official receipts are numbered, dated, and include your name and our clinic's name, address and licence practitioner number.
Chiropodist fees are covered by most private Extended Health Insurance Plans offered by employers, unions, etc. We strongly advise you to contact your insurance provider (or 3rd party payer) to understand your coverage eligibility. You can often find our services listed under your explanation of benefits booklet under the heading "Paramedical Services" for "Chiropodist / Podiatrist services". “Custom-made Orthotics”, “Orthopedic Footwear” and “Compression Stockings” are listed under Medical Equipment or Orthopedic Supplies.

Please know your requirements for any claims with your insurance company (or 3rd party payer). It is highly advised that if you have any questions about your coverage that you contact them directly. You may need to get a medical doctor (M.D.) referral note before you proceed with any service or product with us. As well, some plans may require an estimate letter to be pre-approved for certain products such as orthopedic shoes. If you need an estimate letter, we need to give you an assessment and this usually involves a visit fee. Please call us to inquire, (905) 943-7575 .
