Here are some forms that our established patients may need:

• 2025-2026 AIA Sports Form - if you need the form in less than 24 hours, there is a $25 rush form fee. We must receive this form from you at least 4 hours before you need it completed.
• WIC formula and food request

New Patients

To ensure our practice is the best fit for you family, we ask that you review our practice policies, especially our vaccine policy, and insurances accepted prior to registration:

• The list of insurances accepted is on the "Our Office" page

Registration Steps:

• If you are registering multiple kids in your family at once, follow BOTH steps below:
  • FIRST:  Fill out the Family Registration packet and write the names of all of your children on each page
  • THEN:  Fill out one Individual Patient Registration packet for EACH of your kids (i.e. if you have 3 kids, you will fill this one out 3 times)

• If you are registering for just one patient, fill out one of the following packets:
  • For a newborn, fill out the Newborn Registration packet
  • For a child 1 month of age or older, fill out the Single Patient Registration packet

The Initial Visit

Our pediatricians and staff do whatever it takes to make every visit to our office a pleasant, relaxed experience. With all of the important information about our practice available on our website, you can feel confident that you and your child are well-prepared for your first appointment. Transferring Records to Another Office

If you are moving or changing pediatricians and want to have us send your child's records to another office, fill out our Release of Records Form.
