If your kids are current patients of Dr. Gioannetti and you would like to follow her to Mesquite Pediatrics, we welcome you! We look forward to seeing you here at Dr. Gioannetti's new home starting April 1, 2025. You can call us any time to schedule an appointment and our staff will walk you through the process of getting established with us. The registration paperwork that we will need is below. We would love to get that paperwork completed and in our system before your appointment if possible.
To ensure our practice is the best fit for you family, we ask that you review our practice policies, especially our vaccine policy, and insurances accepted prior to registration:

- We should be contracted with just about all of the plans that Dr. Gioannetti has been taking, with the exception of the Arizona Complete Care AHCCCS plan. We are working with Arizona Complete Care to get contracted but they are currently not accepting new physicians into their network. We encourage you to call them and request that they allow us into their network.
- If you are on any other plan that we do not take, please let us know and we will be happy to look into getting contracted with them.
Registration Steps:
- If you are registering multiple kids in your family at once, follow BOTH steps below:
- FIRST: Fill out the Family Registration packet and write the names of all of your children on each page
- THEN: Fill out one Individual Patient Registration packet for EACH of your kids (i.e. if you have 3 kids, you will fill this one out 3 times)
