Preparing for Your First Visit
Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. Please bring knowledge of your past medical, surgical, and family histories, and current lists of medications you take and doctors you see. Also, please bring with you a valid insurance ID card and a referral, if necessary. We look forward to meeting you on your first visit.
