Welcome to Growing Together Pediatrics, we are delighted you have chosen our office as your child’s medical home. We look forward to caring for your family!
Before we can schedule your child’s first appointment, we ask that you complete a few essential steps to become our patient.
First, please contact your child’s insurance to make sure we are the designated Primary Care Provider or “PCP” on file. Most insurances require this step before any healthcare provider can treat a patient. Please ask the insurance representative for a reference number regarding the PCP change and please give that number to our scheduling staff when making your child’s first appointment. To help you with your PCP change, you may find a list of our providers here.
Next, fill out our New Patient Packet. Once you have submitted this completed packet, you may call us at (407) 770-1414 to schedule your first appointment.
*Please note: New Patients that "No Show" for their first appointment will not be allowed to reschedule.
A few other additional requests and suggestions to make your transition to our practice a great one:
- Bring in a list of your child's medication including the name and dosage of medication, any vaccine records (especially if your child has received vaccines outside the state of Florida), and any important medical records.
- Provide us with the name, address and phone number of your child’s previous doctor or hospital so that we can help you obtain any of the information listed above.
- Bring a copy of your child’s insurance card and your photo ID to their first visit.
- Familiarize yourself with your child's Patient Portal. This is a wonderful, easy to use tool that keeps you connected to our office, gives you access to your child's records, appointments and account with our practice.
